3. HCFA - 1500 BOX 33 PIN# AND GROUP#:

What you want to be shown/sent on paper claims/electronic claims

Go to File ---- Practice Info

Step1:

Step2: Select an insurance carrier from your insurance carrier list.

                CASE 1: Providers with PIN# and Group#. Don't forget to click "OK"

                CASE 2 : Providers with ONLY PIN# or ONLY GROUP#

                    Important: Select Reference Identification Qualifier from drop down list. Click OK to save.

For Ebilling

                CASE 1:  Providers with ONLY PIN# or ONLY GROUP#

                Important: Select Reference Identification Qualifier from drop down list. Click OK to save.

                CASE 2: Providers with PIN# and Group#

                Step1: Check "Doing Business As (DBA)" and enter the group name you've enrolled with insurance carriers and Proxymed

                Step2: Uncheck "Doing Business As (DBA)"

                Step3: Click on "Add" for Provider's Insurance Information

                Important: Check "Use Doing Business As (DBA) For Billing Provider Name
